Who qualifies for Individual Rates?
Individual Rates apply to personal subscriptions ordered or paid for by one individual. Must be for individual use only and paid for by a personal check or credit card of the individual.

What is an Electronic Access Subscription?
An Electronic Access subscription allows multiple users at a single location unlimited access to the Journal online through IP address verification.

When will I receive my first issue?
All subscriptions will begin with the next available issue. Subscriptions may be backdated with payment only, based on availability. Mailing labels are normally produced approximately two weeks before the journal issue date (ex. March issue labels would be generated on February 15).

Does my Institutional Print subscription include online access?
Online access is not included with an Institutional Print subscription. Institutions wishing to access the journal online can do so by purchasing an e-only subscription or by subscribing to e-only plus print. Does my Individual Print subscription include online access?
Yes. Online access to current and archived full-text articles is included with your Individual subscription. To access the Journal online, you must use your 16-digit subscriber number provided with your subscription. Access is available for one concurrent user and is intended for individual use only.

Online access will begin when you receive your first print issue from SLACK Incorporated.

How do I access articles online with my Individual print subscription?
Online access to Journal articles is available with all Individual subscriptions. Individual subscribers may access the Journal online using the subscriber number provided with your subscription. Your 16-digit subscriber number can be found above your name on the mailing label of your print copy (e.g. #1234567890123456#). Please note "#" is not a part of your 16-digit subscriber number.

Are there shipping charges for subscriptions shipped outside of the U.S.?
All orders shipping outside the United States are sent by air service. Subscriptions mailing to addresses outside of the U.S. will include the appropriate shipping charges. If your subscription is being mailed to Canada, please include the 5% GST. SLACK Incorporated's GST number is 129780466.

SLACK Incorporated maintains three years of back issues. We accept backdated subscriptions at the institutional rate only. Backdated orders are subject to availability and must be prepaid.

How do I claim a missing or damaged issue?
Claims for missing and damaged journals must be received by SLACK Incorporated within 90 days of publication date for journals mailing within U.S. and 120 days for journals mailing outside the U.S. Any claims received outside the claim period will not be processed.

Can I purchase individual articles on a pay-per-view basis?

Does SLACK Incorporated offer a discount to subscription agents?
SLACK Incorporated offers a 5% discount off the published medical journal rates to qualified subscription agencies. Additional cost for postage outside the U.S. and Canadian GST are not discountable.

Does SLACK Incorporated offer a discount on its journals or newspapers to residents or students?
Resident/Student discounts are available for most of SLACK Incorporated's journals and newspapers. All Resident/Student subscription orders must be accompanied by a letter from the academic organization of study and are subject to verification.
